7th Grade Immunizations

Please be reminded that there are two immunizations required by law for your child to attend school this academic year.

Tdap (tetanus, diptheria, pertussis) one dose

MCV (meningococcal) one doseThese immunizations are required by NC law on or before the first day of school and proof of immunization must be provided. Immunization records can be faxed to Mrs. Long's attention at 704-237-5398 or can be turned into the Wellness Office beginning If you send a hard copy, please be sure to also keep a copy for your records. If you have not provided the school with proof of immunization by the first day of school, you will be given 30 days to provide this information. After, students without proof of immunization will not be able to attend school until they are immunized per NC law.

Please refer to www.immunize.nc.gov for more information or contact Rebecca.Long@pinelakeprep.org with questions. Thank you in advance for your assistance.
